Radiofrequency in Medicine

Radiofrequency nerve ablation is used by interventional anesthesiologists to treat chronic back and neck pain, including pain from facet joint dysfunction, sciatica, nerve impingement, intervertebral disc dysfunction, sacroiliac joint dysfucntion, headaches, postoperative pain, complex regional pain syndrome (CRPS), reflex sympathetic dystrophy (RSD), and other disorders.  Podiatrists employ the same technology to treat intractable pain due to foot and ankle disorders, including plantar fasciitis and Morton’s Neuroma.  Neurosurgeons employ radiofrequency in the brain to treat movement disorders such as Parkinson’s Disease, essential tremor, and spasticity.  Neurosurgeons also use RF to treat severe, intractable pain such as that arising from end-stage cancer (cordotomy), nerve root avulsion injury (DREZ and DREZ nucelus caudalis), and trigeminal neuralgia (PSR).

Radiofrequency is most often used in its “thermal” form, whereby high intensity RF currents heat target tissue near the RF electrode to destructive levels.  In pain management, the target tissue may be a nerve that carries painful impulses.  Radiofrequency is also used in a “pulsed” form, whereby RF currents are delivered in short bursts and controlled such that the measured gross tissue temperature does reach levels expected to product thermal damage. Nerves adjacent to points of high curvature on a PRF electrode’s surface are intermittently subjected to transient temperature spikes within the neurolytic range, whereas nerves in the gross vicinity of the PRF electrode are exposed principally to high electric fields, which may produce adisruptive or neuromodulatory effect.
